Sounds like his OCD is pretty severe. He needs to be properly diagnosed and put on stabilizing medication. Instead of just blurting out "Go or else" explain that he needs to get it under control now before it gets worse. Maybe a family or friends intervention might coerce him if he's refusing to go when you ask him.
